Facility Use Agreement - Addendum
Groups, organizations, or individuals using The Oconomowoc Area School District’s facilities (hereinafter designated as “Facility User”) expressly agrees and understands that the Facility User is entering The Oconomowoc Area School District at their own risk to engage in the activities as stated by the formal request on the Facilities Use Agreement. The Facility User understands the novel coronavirus, COVID-19, which has been declared a worldwide pandemic by the World Health Organization (WHO).
The Facility User hereby agrees, represents, and warrants to not enter or allow participants/members to enter District property if they (i) experience symptoms of COVID-19, including, without limitation, fever, cough, or shortness of breath; (ii) have a suspect or diagnosed/confirmed case of COVID-19; or (iii) are currently within a quarantine period for close contact exposure to COVID-19. The Facility User also agrees to notify all participants/members of the aforementioned requirements and implement a means to ensure understanding and compliance. The Facility User may require appropriate waivers for members/participants (adults and minors), and it is the responsibility of the Facility User to keep all waivers for at least two years and make them available to the District upon request.
It is further understood that the Facility User has the responsibility to determine and enforce COVID-19 mitigation measures for its participants. This is the full responsibility of the Facility User and the District at no time will take on any ensuing liability.
If the Facility User is notified or becomes aware of the fact that one or more of its group contracts Covid-19, or had Covid-19 while being on District property, they MUST immediately (as soon as practical but no later than 24 hours of initial knowledge) report this to the District, as well as to local health officials as required by State and local health authorities. If the District needs to shut down for a short time period because of the need to sanitize due to a positive Covid-19 test on premise, they will not be responsible for any loss of revenue to any organization whatsoever.
The Oconomowoc Area School District reserves the right to change and enforce practices and policies for facility utilization as circumstances of COVID-19 arise, be it from a local, State, or Federal mandate or any other reasonable circumstance or situation.
